We are an in-network provider for BCBS PPO and BCBS Choice plans. We can work with any insurance, but it may be at your out of network benefits.
FEE FOR SERVICE / CASH RATES
Initial Evaluation: $185
Follow-Up Treatment Sessions: $150
Discounted Follow-Up Package of Four Sessions : $560
Out Of Networking insurance
If you subscribe to another insurance policy, we can still provide physical therapy services for you, however, it will be at your out of network benefits. We can help you navigate estimating costs associated with an out of network provider.
Medicare
We are an out of network provider for Medicare which means we can treat Medicare patients however you will be responsible for what medicare does not cover. We can assist you in determining your balance with your Medicare cap.
